Monthly statements for PFA Models
Authorized individuals must submit monthly statements to the 25th of next month to achieve income.
The statement that is submitted to the Pension Fund by individuals can be downloaded at:

ie, insurance statement provided in Article 7 of Annex 2 of the GD 791/2010
The statement that is submitted to the House of Health Insurance for individuals can be downloaded at:

The statement that is submitted to unemployment by individuals can be downloaded at:
http://www.bucuresti.anofm.ro/oug58.htm
Failure to submit declarations is fined between 2500-5000 USD.
Because it is very busy at the counters where these statements shall recommend sending them by mail at the following addresses:
